Health & Physical Forms

Please review the instructions and required forms here. Please read the directions carefully and complete the forms that pertain to your child. You will need to submit 3 separate online forms on the portal: “Entrustment of Care Form”, “Student Health Update”, and “Student Forms & Health Records Submission”.  

Be advised, some forms require a physician’s signature, so it is recommended you review the forms now so you can plan accordingly. To expedite online submission, you may ask your physician for a scanned electronic copy (PDF preferred) of the physical evaluation forms/sports clearance and your child’s vaccine records. 

All form submissions are due by Thursday, August 1st, 2024. For students participating in a fall sport, they are due on the first day of practice. Please note that without these forms, students are unable to participate in extracurricular activities.

We will also accept mail-in submissions of these forms, but online submission is preferred.   

All printed form submissions must be returned via USPS mail.  For security and privacy reasons, do not email completed forms. Because our campus is currently closed to visitors, drop-off of forms is not available.
